DR. JOHN R. HOFFMAN TAKEN BY DEATH MONDAY Dr. John R. Hoffman, secretary of the Chicago Eye, Ear, Nose and -Throat hospital since it was founded in 1897, vice president of the Chica- go Ophthamological society and a prominent resident of Wilmette since 1911, died at his residence, 1231 Green- wood avenue, Monday morning, Feb- ruary 19^ at 10:45 o'clock. Dr. Hoffman was well known to residents of Wilmette and always took a keen interest in the affairs of the village. He was born at Ottawa, 111., in 1865. He attended John Hopkins univer- sity and was a graduate of Rush Medical college. He practiced for a time at Ottawa and removed to Chi- cago in 1897. At the time of the Spanish American war he was sur- geon in the 3rd Illinois^ Infantry. He is survived by his widow and three daughters-, Phoebe, Faith and Patricia. His son died in the service.
